pg_reorg 1.0.4 supports PGXS and fixes some bugs.

Changes:
- Support of PGXS.
- Fix ownership bug. New toast table, toast index, and toast type should not have be owned by executor of pg_reorg, but by the original owner.
- Modify LOCK to LOCK-NOWAIT-and-loop at swapping tables not to block other transaction when exclusive-locking takes long time.
